Review important information about GLP-1 treatment, sign your name, and introduce yourself to your provider.
Please read the following about GLP-1 medications
Important Information About GLP-1 Agonist Treatment
GLP-1 receptor agonists (such as Semaglutide and Tirzepatide) are medications that mimic the GLP-1 hormone to regulate appetite and blood sugar. Common side effects include nausea, vomiting, diarrhea, constipation, and decreased appetite. Serious but rare side effects may include pancreatitis, gallbladder problems, kidney issues, and allergic reactions.
GLP-1 medications should NOT be used if you have a personal or family history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). These medications have caused thyroid C-cell tumors in rodents. It is unknown whether they cause thyroid tumors in humans.
You should inform your provider about all medications you are currently taking. These medications may affect how other drugs work, particularly insulin and sulfonylureas. Do not use if pregnant or planning to become pregnant.
By consenting below, you acknowledge that you have read and understood the above information, including the potential risks, benefits, and alternatives to GLP-1 treatment.
